Nurse Practitioner - Primary Care

Nurse Practitioner PositionThis position is located at the McMinnville Outpatient Clinic in McMinnville, TN. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties include:Independently and responsibly managing a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practiceDeveloping and implementing an individualized plan to achieve optimal population health and disease managementDemonstrating leadership and working collaboratively with the Primary Care Team and other interdisciplinary team membersPerforming histories and physical examsProviding appropriate documentation to minimally include history, physical exam findings, diagnosis, definitive care plan and disposition of the patientEnsuring that patient encounters are appropriately closed out including disposition that clearly indicates the next step of careEnsuring that each patient is provided with a list of current medications and an appointment list at close-outCommunicating test results to patientsOrdering and interpreting labs, tests, and diagnostic studiesPrescribing pharmacologic and non-pharmacologic interventionsInitiating referrals and consults to specialty servicesProviding education to patients and their familiesAssessing and adjusting the plan of care as needed to achieve optimal patient outcomesExemplifying the ICARE core values of Integrity, Commitment, Advocacy, Respect, and ExcellenceDemonstrated experience leading clinical quality improvement initiatives or managing healthcare programsBackground in project management within a clinical setting strongly preferredTraining or certification in quality improvement methodologies (e.g., Lean, Six Sigma) is a plusProven ability to implement and sustain process improvements in primary care or outpatient environmentsOther duties as assignedVA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Monday - Friday 8:00am-4:30pmTelework: May be eligible for ad hoc teleworkEDRP Authorized: Former EDRP participants ineligible to apply for incentive.
