As Needed Hardinsburg, KY, US 5 days ago Requisition ID: 1391 Responsibilities Maintain and enhance the organization's Department of Pharmacy by involvement in the selection and procurement of medications, storage of medications, medication order entry, preparation and labeling of medications, dispensing of medications, and medication process quality improvement activities. Ensure all duties intended to properly identify each patient are completed. Prepare and dispense medication orders per physician request according to established policies, procedures, and protocols. Interpret medication orders accurately and maintain complete patient medication profiles. Compound and accurately dispense pharmaceuticals, including sterile products. Issue controlled substances to patient care areas and maintain records as required by law. Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. Participate in the pharmacy pharmacokinetic dosing service and drug dosing service per organizational protocol. Review/intercept culture and sensitivity data for antibiotic appropriateness and recommend changes as needed. Participate in the pharmacy clinical program which includes medication profile review and the ability to accurately read, extract, and interpret patient chart information. Document clinical activities and interventions accurately and completely. Detect and report suspected adverse drug events and medication errors accurately and in a timely fashion. Sustain the formulary by minimizing non-formulary procurements by utilizing therapeutic substitution protocols and promoting rational drug therapy selection. Provide clinical consultation and clarification to practitioners, suggest appropriate cost-effective therapeutic alternatives to medical staff as needed, and provide accurate, adequate, and timely drug information to the professional staff. Qualifications Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy. State pharmacist licensure in good standing. Experience in a hospital or clinical setting is preferred. Strong knowledge of pharmacology and drug therapeutics. Excellent communication skills for collaborating with a multidisciplinary team. High level of accuracy, attention to detail, and ability to work independently. As needed for vacation and sick relief. No weekends or on-call required. #J-18808-Ljbffr Breckinridge Health, Inc.
