Physical Therapist

Eastside Rehabilitation Hospital seeks a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate and treat patients with diverse conditions, developing individualized rehabilitation plans to restore function, mobility, and independence. Collaborate closely with an interdisciplinary team, educate patients and families, and document progress. Responsibilities Evaluate patients with musculoskeletal, neurological, and post-surgical conditions through comprehensive physical therapy assessments. Develop and implement individualized, evidence-based treatment plans to improve patients’ mobility, strength, balance, and functional independence. Provide hands‑on manual therapy, therapeutic exercise, gait training, and neuromuscular re‑education tailored to each patient’s needs and goals. Educate patients and their families on home exercise programs, safe mobility techniques, and strategies to support ongoing recovery. Collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team to coordinate holistic patient care. Document all assessments, treatments, progress notes, and discharge summaries accurately and timely in the electronic medical record system. Monitor and adjust treatment plans based on patient progress, clinical judgment, and evidence‑based best practices. Ensure safe and appropriate use of rehabilitation equipment and modalities in accordance with hospital policies and professional standards. Participate in case conferences, team meetings, and quality improvement initiatives focused on enhancing patient outcomes and care delivery. Support and mentor therapy students or new staff as appropriate, contributing to a culture of continuous learning. Uphold all regulatory, ethical, and hospital standards for patient privacy, infection control, and workplace safety. Graduation from an accredited Physical Therapy program (CAPTE‑accredited). Basic Life Support (BLS) certification required or obtained upon hire. Advanced certifications such as Neurologic Clinical Specialist (NCS), Orthopaedic Clinical Specialist (OCS), or Certified Stroke Rehabilitation Specialist (CSRS) preferred but not required. #J-18808-Ljbffr Stryker Corporation
