On Call Registered Nurse
Heartland Hospice
Heartland Hospice is seeking an On Call Registered Nurse to provide after-hours hospice care to patients in home and facility settings. The RN triages calls, makes emergency visits, manages symptoms, and implements care plans in collaboration with the interdisciplinary team. Responsibilities include pain and symptom management, patient and family education, coordination with physicians, accurate documentation, and support during end-of-life transitions. The ideal candidate is compassionate, autonomous, calm under pressure, and committed to providing dignified, patient-centered care during evenings, nights, weekends, and holidays.
