Job Description
Title: Physical Therapist - Home Health

Schedule:
- Weekends: None required
- Holidays: If one major holiday falls within the assignment, expected to work it. If two major holidays, expected to work both. If three major holidays, expected to work two.

Assignment Details:
- Daily census: Approximately 80 patients
- Typical visits per day: 6

Requirements:
- Minimum experience: 1 year
- Required certification: BLS
- Charting system required: HomeCare HomeBase (HCHB)

Roles & Responsibilities:
- Conduct home health assessments and document using OASIS guidelines.
- Develop and manage a plan of care and conduct reassessments as required.
- Provide rehabilitation for orthopedic and neurologic conditions including post-operative care.
- Assist patients with mobility and transfers, including gait training and use of assistive devices.
- Educate patients and families regarding discharge planning and safety considerations.
- Monitor skin integrity and wound awareness.

Compliance / Notes:
- Orientation: 40 hours; number of non-billable orientation hours determined by contract length.
- Requested Time Off (RTO): Maximum of 7 days per contract.
