Clinical Pharmacist Ambulatory Rheumatology

Job Description Join our Intermountain Health Ambulatory Rheumatology Pharmacy Team as a clinical pharmacist practicing under a collaborative practice agreement, delivering compassionate, evidence‑based, high‑quality, patient‑centered care to rheumatology patients in an outpatient clinic setting. Schedule: Monday – Friday, no weekends or holidays. Shift: Day shift with expected work hours based on clinic hours and patient care needs. Location: Saint Joseph Hospital, Denver, Colorado. This position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient and cost‑effective pharmaceutical care within the scope of their assigned practice. The position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team. Essential Functions Review patients’ medication orders for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug‑drug interaction. Make evidence‑based interventions to optimize medication use. Ensure safe, appropriate, cost‑effective therapies and accurately dispense medications. Oversee and/or participate in assuring appropriate medications are prepared and delivered timely to care areas and are stored securely and appropriately for both stability and effective workflow. Provide counseling to patients and promote quality medication education and adherence. Ensure patients are well informed, treated with respect, and receive the highest level of professional service. Complete all regulatory and compliance training and ensure documentation of regulatory requirements is clear and complete. Be knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures. Supervise and direct activities of interns, technicians, and support personnel as required. Provide medication‑related clinical consultation and clarification to other health professionals. Takes the initiative to solve problems, including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns. Identify potential areas for improvement, including processes that could be streamlined or revised to improve patient satisfaction. Identify potential safety hazards that may create problems for patients and/or staff and take appropriate actions. Participate in departmental meetings, contributing ideas or feedback. Support clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service. Support local and enterprise pharmacy leadership initiatives. Skills Pharmacy Pharmacy Services Medication Management Pharmaceutical Care Project Management Quality Improvement Problem Solving Federal Pharmacy Laws State Pharmacy Laws Qualifications Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified). License to practice pharmacy in the state of practice and dispense controlled substances. A Colorado pharmacist license is required before hire. Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice. Effective communication, people management, and project management skills. Immunization certification if applicable to practice site needs. Preferred: Previous experience working in an integrated healthcare delivery system. Preferred: Board Certification. Preferred: Health System Residency. Physical Requirements Ongoing need to see and read information, labels, documents, monitors, identify equipment and supplies, and assess customer needs. Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require verbal communication and hearing and understanding spoken information, needs, and issues quickly and accurately. Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc. Scheduled Weekly Hours: 40 Hourly Rate: $62.89 – $92.44 (actual rate dependent upon experience).
