About the Role
This is a Home Health Physical Therapist Case Manager position supporting patients in the Central Metro A territory. The PT will manage their own caseload, provide skilled home-based therapy, and coordinate care to help patients safely recover and maintain independence at home. This role offers a steady weekday schedule with no on-call, weekends, or holidaysideal for therapists who value consistency and autonomy.
Key Responsibilities
Perform in-home physical therapy evaluations and treatments
Develop and implement individualized plans of care
Act as the case manager for assigned patients, coordinating services as needed
Educate patients and caregivers on mobility, safety, and home exercise programs
Complete timely and accurate documentation per home health standards
Communicate effectively with physicians, nurses, and interdisciplinary team members
Ensure compliance with regulatory and agency requirements
Patient Population
Adult and geriatric home health patients
Typical diagnoses include post-acute recovery, mobility impairments, and chronic conditions requiring skilled PT
Required Qualifications
Active Physical Therapist license in Illinois
Home Health PT experience strongly preferred
Strong clinical judgment and time-management skills
Comfortable working independently in a home-based setting
Floating & Travel
No floating required
Coverage limited to the assigned Central Metro A home care territory
