Work Schedule Hours: 7:00 AM – 3:30 PM, 16 hours every 2 weeks (0.2 FTE). Every fourth weekend is a holiday rotation. Pay RN Pay: $41.29–$564.16 per hour. LPN Pay: $24.86–$35.84 per hour. Job Description Summary Delivers nursing care by developing a nursing diagnosis; establishing nursing care objectives; defining nursing interventions; providing physical, emotional, psychological, and spiritual support to residents, families, and friends to understand residents conditions, medications, and self‑care skills; noting changing conditions and needs; re‑evaluating care objectives. Required Education, License/Certification, or Work Experience Registered Nurse (RN) – Board of Nursing. An active license in the state of practice upon hire. Preferred Education, License/Certification, or Work Experience 1–3 years’ experience working in a clinical or long‑term care environment. Brief Overview The Licensed Practical Nurse in Long Term Care is accountable for providing each resident with professional nursing care, assessment, and evaluations in their scope of practice. What you will do Perform all licensed practical nursing functions as directed by the Registered Nurse to all residents. Complete assessments on residents, make appropriate interventions, apply knowledge, prioritize resident care, using the nursing process (assess, plan, implement, evaluate). Comply with all pharmaceutical policies and procedures, ensuring that all medications/treatments are given safely and accurately in accordance with physician’s orders. Participate in the care‑planning process and assure that resident care is implemented consistently and appropriately to the age of those served; initiate and update the plan of care as appropriate. Comply with standards of professional documentation. Communicate and manage the delivery of resident care, ensuring quality care is provided. Facilitate conflict management while respecting diversity of people, strengths, and ideas. Guide neighborhood staff in meeting the needs of the residents, practicing routines based on residents’ needs. Participate in physician rounds, communicate changes in resident condition to physician using the SBAR communication tool. Process and transcribe physician’s orders and implement according to facility policy. Function within the scope of the Nurse Practice Act, utilizing recognized standards of clinical practice as defined by the State of MN. Comply and enforce facility staffing policies. Assist with data collection for quality functions and performance‑improvement activities. Seek educational opportunities, share knowledge with others, assist with orientation of new staff. Demonstrate appropriate decision‑making skills. Maintain competency with operational equipment. Investigate incidents, patient/family complaints and make immediate report to VP of Resident Care or Administrator on‑call. Administer medications according to facility policy, competent in the Pyxis medication administration system. Assist in discharge planning and education. Perform all other duties as assigned. Essential Qualifications The individual must be able to work the hours specified and perform each essential job function satisfactorily, including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds. An active license in the state of practice upon hire. Preferred Education, License/Certification, or Work Experience: LPN 1–3 years of long‑term care nursing services.
