Registered Nurse (RN) – Per Diem Medicare Home Health | OASIS Experience

Dedicated Home Health is growing and is seeking experienced Registered Psych Nurses (RNs) to join our Medicare Home Health team on a per-diem basis.

What We're Looking For

Medicare Home Health and OASIS experience are strongly preferred.

Qualified candidates should have:

• Current, unrestricted Florida Registered Nurse (RN) license
  
  
  
• Medicare-certified Home Health experience
  
  
  
• Hands-on OASIS assessment experience

• Must have Psych experience
  
  
  
• Experience completing Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits
  
  
• Strong head-to-toe assessment and clinical documentation skills
  
  
• Understanding of Medicare home health requirements and plan-of-care coordination
  
  
• Ability to communicate effectively with physicians and the interdisciplinary care team
  
  
• Reliable transportation and ability to travel within the assigned territory
  
  
• Commitment to timely documentation and high-quality patient care
  
  

The Role

As a Per Diem Home Health Registered Nurse , you will provide skilled nursing care to patients in their homes while coordinating care with physicians, therapists and other members of the interdisciplinary team.

Responsibilities may include:

• Completing comprehensive OASIS assessments
  
  
  
• Start of Care and other skilled nursing visits
  
  
• Medication reconciliation and patient/caregiver education
  
  
• Disease-process and chronic-condition management
  
  
• Wound assessment and treatment, when ordered
  
  
• Monitoring changes in patient condition
  
  
• Communicating significant findings to physicians and the clinical team
  
  
• Updating and coordinating the patient’s plan of care
  
  
• Completing accurate and timely clinical documentation
