Discharge Planner
Shift Information
Monday to Friday, 8:00 a.m. to 5:00 p.m. with one-hour lunch (8-Hour Shift)
Minimum Experience
Two (2) years of experience in discharge planning and medical-surgical settings is required.
Certifications/Requirements
- Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
- American Heart Association (AHA) certification in Basic Life Support (BLS) is mandatory and must be maintained.
- General certifications (ACLS, BLS/BCLS, PALS, NRP, ATT) are required.
Job Description
The Discharge Planner performs case management and coordinates the discharge planning process for inpatients, focusing on those with chronic diseases and special healthcare needs. This includes working in various units such as the Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit. Responsibilities include:
- Maintaining a tracking system for patients referred for admission to other healthcare facilities.
- Conducting chart reviews for requested services related to patient discharge or return to the facility.
Schedule and Orientation
All travel staff are required to attend New Employee Orientation, scheduled every other Monday. Travelers must be cleared seven days prior to their start date. If not cleared, the start date will be deferred to the next scheduled orientation.

No holiday coverage is required for this position.
