Working with the Nurse Care Manager, you will conduct daily patient follow-ups, process DME and home health orders, triage calls, and retrieve hospital records for recently admitted patients. You will collaborate with physicians and the care team to ensure high-quality, patient-centered care for high-risk patients, identify barriers to care, and help guide patients through the continuum #J-18808-Ljbffr Jobless
