Home Health Hospice RN

Setting: In-office processing referrals for home health and hospice. Shift Notes: 8:30 AM to 5:00 PM, every other weekend. Coverage Area: Northern California Home Health or Hospice. Average Miles per Day/Week: 10%. Common Equipment & Charting System: Health Connect (Epic).
