Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and medical record reviews to identify patients' emotional, social, and environmental challenges affecting their rehabilitation journey.

• Collaborate closely with a multidisciplinary rehabilitation team to develop and implement individualized treatment plans that prioritize patients' psychosocial needs, ensuring integrated support across disciplines.

• Assist patients in navigating community resources, including support groups, housing assistance, and vocational rehabilitation services, to promote their overall well-being and independence.

• Educate patients and families about community support mechanisms available post-rehabilitation, empowering them with the knowledge to seek necessary assistance.
