Physical Therapist Assistant - PTA (Constellation Home Care) Location: Smithtown, NY 11787. Position Type: Full Time. Salary Range: $72,000.00 - $75,000.00. Constellation Home Care is looking for a Physical Therapist Assistant (PTA) to join our home care team. This field‑based role requires the PTA to provide physical therapy services under the supervision of a Registered Physical Therapist, with a good standing license, to patients in their homes. Services are delivered in accordance with a written Plan of Care established by a physician. Responsibilities Provides skilled Physical Therapy services. Initiates appropriate preventive and rehabilitative physical therapy procedures. Teaches and supervises the patient and family in the home environment. Coordinates physical therapy with all other professional services involved in patient care. Provides direction and supervision to the home health aide regarding simple physical therapy exercises and techniques when appropriate. Is supervised by a Physical Therapist at least every thirty (30) days, with conference documented in the patient’s clinical record, reviewing the current care plan and any modifications to the treatment regime. Initiates, reviews and implements the PT Care Plan for each patient based on assessment and prescribed Plan of Care, revising when indicated. Observes, evaluates and reports on the patient’s physical and emotional status to other team members and the attending physician whenever a change occurs, including reaction to treatment and medications. Provides patient progress reports to the attending physician whenever there is a change in condition or at least every 60 days. Maintains clinical records for each patient receiving care, including observation, treatments, response to treatment, teaching activities, changes in status, indications of progress, proposed continuing care plan, justification of continuation, and other pertinent data. Qualifications Graduate from an accredited physical therapy assistant educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE). Licensed to practice as a Physical Therapist Assistant - PTA in the state of employment. Must meet all requirements for regulatory compliance in good standing. Has a minimum of one year of clinical experience. Previous home care experience preferred. Have reliable transportation and be willing to commute to provide the best in‑home patient care.
