Housing Stability Case Manager

Avenue 360

Avenue 360 is seeking a Continuum of Care Housing Case Manager in Houston to support stable housing and prevent homelessness. You will conduct assessments, develop service plans, coordinate with property owners, and ensure accurate documentation within HMIS. Requires a bachelor’s degree or commensurate experience with 2+ years in case management or housing programs. Strong communication, organizational, and multitasking skills are essential to navigate complex client needs and funding #J-18808-Ljbffr
