Housing Case Manager

Avenue 360 Health and Wellness

Avenue 360 Health and Wellness is seeking an Outreach Navigator/Assessor to identify and connect individuals experiencing homelessness with housing resources through the Coordinated Entry System. The role includes outreach, housing assessments, HMIS data entry, and assisting with documentation for PSH and RRH. Preferred candidates will have a Bachelor's degree in a related field and at least two years in social services or homelessness outreach, with strong communication and organizational #J-18808-Ljbffr
