Humana Inc. is seeking a Telephonic Nurse 2 Care Manager to support SNP members from a remote home office. You will assess health, coordinate care, and remove barriers through telephonic outreach, guided by a multidisciplinary team. Training lasts four weeks with EST hours; bilingual Spanish/English required; high emphasis on privacy and compliance.
