Key Responsibilities Perform home health physical therapy evaluations and develop care plans Provide therapeutic treatment and instruct patients/caregivers Monitor progress and report changes to physicians Participate in discharge planning and interdisciplinary case conferences Supervise Physical Therapy Assistants per state regulations Qualifications Current California Physical Therapy License 2+ years of physical therapy experience CPR certification Valid driver’s license and insured vehicle Why GrandCare?
