Senior Case Manager for Older Adults & Community Programs

Catholic Charities Brooklyn and Queens, Inc. seeks a Case Manager II to assess senior needs and coordinate services under the Program Manager. You will develop care plans, arrange information and referrals, and maintain records in line with agency regulations. The role includes leading biweekly group discussions and delivering monthly benefits workshops for seniors, along with coordinating special events and activities.
