Job Overview

This pharmacist is a member of the Department of Pharmacy with a primary focus of providing comprehensive pharmacy services to patients admitted to Tufts Medical Center with 60; Advanced Heart Failure , including patients before, during, and following heart transplantation and left ventricular assist device (LVAD) implantation. This pharmacist will work with other clinical pharmacists, including working in tandem with an 60;additional 60;inpatient Advanced Heart Failure pharmacist, and with the broader multidisciplinary teams to ensure 60;optimal 60;patient care. The pharmacist in this role will provide leadership for implementing and 60;maintaining 60;drug policy guidelines and protocols. This pharmacist will 60;be responsible for 60;maintaining 60;a clinical practice while educating Tufts Medical 60;Center 60;pharmacists, pharmacy residents, pharmacy students, and other care team members. 60;

Responsibilities Include 60;

• Working with members of the pharmacy, medical and surgical staff to care for advanced heart failure 60;patients including heart transplant recipients and LVAD patients. 60;

• Working with inpatient pharmacists to ensure 60;optimal 60;operational and clinical pharmacy services to 60;every patient for whom they have responsibility, including review and approval of medication orders

• Participating in multidisciplinary rounds with the Advanced Heart Failure Team(s) 60;

• Completing pre-transplant evaluations for prospective heart transplant and LVAD recipients. 60;

• Evaluating and documenting post-operative medication reconciliation and the drug therapy plan for heart transplant recipients. 60;

• Participating in and documenting discharge medication reconciliation and patient education. 60;

• Completing index heart transplantation medication discharge teaching and index LVAD implantation 60;warfarin/enoxaparin teaching in compliance with CMS regulations 60;

• Facilitating transitions of care between cardiology services (i.e., inpatient and outpatient). 60;

• Working with members of the heart failure and transplant team to 60;facilitate 60;patient access to medications. 60;

• Providing support, in cooperation with other clinical pharmacists, to outpatient cardiology clinics. 60;

• Maintaining selected institutional guidelines, 60;protocols 60;and policies. 60;

• Ensuring compliance with quality and regulatory standards. 60;

• Participating in weekly Heart Transplant Listing and LVAD meetings, monthly Quality and 60;Performance Initiative (QAPI) meetings, and morbidity and mortality conferences. 60;

• Serving as a preceptor for PGY1 and PGY2 residents and pharmacy students. 60;

Job Description

Minimum Qualifications:

1. Doctor of Pharmacy (PharmD) 60; AND Two (2) years of clinical pharmacy experience in a hospital 60; OR Bachelor’s Degree in Pharmacy 60; AND Five (5) years of clinical pharmacy experience in a hospital

2. Pharmacist License.

Preferred Qualifications:

1. Doctor of Pharmacy (PharmD) 60; AND Three (3) years of clinical pharmacy experience in a hospital 60; OR Bachelor’s Degree in Pharmacy 60; AND Seven (7) years of clinical pharmacy experience in a hospital.

2. PGY1 Pharmacy Residency.

3. PGY2 Residency in a relevant area of practice.

4. Pharmacy Board Certification.

5. Active Board of Pharmacy Specialties (BPS) 60;credential 60;in a related area of practice.

6. Experience in drug 60;utilization 60;review and analysis.
