Discharge Planner- Contract Role

Detail oriented in completing the psychosocial assessments. Discusses the goals and implementation of treatment plans, progress toward goals and develop an appropriate transition plan upon admission. Provide family sessions per protocol. Provide scheduled group therapy and when needed, individual therapy. Responsible for proper placement and appointments required prior to discharge. Formulates appropriate diagnosis in terms of the current edition of the DSM and ICD, based on pertinent available information. Consults with medical doctors and/or treatment team members concerning treatment goals and objectives. Completes all required paperwork and maintenance of client files according to all applicable regulatory standards. Performs duties as a court liaison. Perform other duties as required by Supervisor. Core Competencies and Qualifications Job Knowledge Problem Solving Interaction with Others Initiative Customer Service Attendance Reliability Cooperation and Interpersonal Skills Education and Experience Requires a Bachelor’s in Psychology or Mental Health Related field. Two (2) years' experience in the mental health field. Computer Skills Intermediate to advanced experience in MS Office Suite (Outlook, Word, Excel, PowerPoint), use of personal computers, and standard office productivity software. Language Skills Ability to read and interpret documents such as safety rules, operation and maintenance instructions, and procedure manuals. Requirements A valid Class E Drivers’ License for positions that require driving personal or Agency vehicle on Agency business. Must maintain a minimum Bodily Injury & Property Liability insurance limit of at least $50,000 for positions that require driving personal own vehicle (POV) on Agency business. (Proof Required) PPBH is a 24/7 Agency. All employees must be available as needed when required.
