Physical Therapist (PT) - Home Health

We are seeking a dedicated Physical Therapist (PT) for a rewarding opportunity in home health services. The ideal candidate will provide compassionate patient care while working independently in various home settings.

Position Overview

• Start Date: ASAP
• Shift: 8-hour days (8:00 AM - 4:30 PM)
• Patient Ratios: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
• Weekend Requirements: No
• On-Call Requirements: No
• Local Candidates: Accepted within a 50-75 mile radius
• Pending License: Accepted
• Guaranteed Hours: Facility may cancel up to one shift per pay period

Qualifications

• Experience: Minimum of 1 year of skilled home health experience required
• Certifications: Basic Cardiac Life Support (BCLS) required
• OASIS Experience: Required
• Transportation: Must have reliable transportation
• COVID-19 Vaccination: Primary series plus one booster required

Special Requirements

The position demands flexibility and effective time management skills as the professional will travel to various patient locations. The facility strives to minimize drive time by grouping clients effectively, though occasional travel distances may exceed 50 miles.

Charting

Charting will be done using EPIC and OASIS systems.

Attire

Scrubs of any color are acceptable for this role.
