This Home Health Physical Therapist position is based in Medford, Oregon, a welcoming city in the heart of Southern Oregon. You will travel to patient residences, conduct thorough evaluations, develop individualized treatment plans, and deliver one-on-one interventions designed to maximize safety, mobility, and independence in the home setting. Typical patients may include adults and older adults recovering from surgery or hospitalization, individuals managing chronic conditions, and those needing support to improve functional status and reduce fall risk.A typical day includes reviewing the schedule, driving to patient homes, completing assessments and treatments, updating documentation in the EMR system, and coordinating with other members of the home health team. You will perform gait training, transfer training, balance and strengthening programs, home exercise instruction, and home safety assessments. You may recommend adaptive equipment or environmental modifications to enhance patient safety and comfort. Caseloads are structured to allow for focused, patient-centered care, with time allotted for travel and documentation.You will be part of a collaborative, supportive clinical environment that values professional autonomy and encourages ongoing development.
