Geriatric Nurse Practitioner - In-Place Long-Term Care

Longevity Health

Longevity Health is seeking an Advanced Practice Provider (NP or PA) to manage a panel of nursing home residents under the Institutional SNP Model of Care. The APP will deliver collaborative primary care, develop individualized care plans, and participate in facility care teams to align with member goals. Responsibilities include coordinating with PCPs, handling prior authorizations, reviewing medications, and closing quality gaps (HEDIS/STARs) while maintaining strong communication with #J-18808-Ljbffr Longevity Health
