Work Environment

This full-time position is primarily onsite, delivering home care therapy. You will operate in various residential settings, adapting approaches based on each patient's unique living situation and specific health requirements. Key Responsibilities

• Conduct comprehensive Physical Therapy evaluations for children and adults, assessing their physical capabilities, mobility, and specific needs.
• Develop and implement clear, individualized treatment plans that align with each patient's rehabilitation goals and address their unique challenges.
• Continuously monitor and reassess clinical signs and symptoms to gauge treatment effectiveness, make necessary adjustments, and set new goals aligned with patient progress.
• Complete all documentation meticulously, including SOAP notes, progress summaries, and care plans, utilizing the company's electronic health record system within established deadlines.
• Communicate effectively with patients and their families, providing education on recovery processes and addressing potential challenges during rehabilitation.
• Meet minimum utilization goals and collaborate with peers across disciplines to enhance service delivery, particularly for patients engaging in multiple therapy services.
• Embark on community outreach and educational projects, sharing knowledge and resources that uplift both patients and the local community.
• Champion the company's values and philosophy, acting as a representative of Ensemble Therapy through professionalism and patient advocacy.

Patient Population / Client Focus

Your expertise will be directed toward a diverse clientele spanning various demographics, including age, race, and socioeconomic backgrounds. This role includes providing therapy to children with developmental disorders, elderly patients recovering from surgery, and individuals with chronic pain or mobility limitations. Your adaptability is vital as you modify treatment approaches based on the specific needs and learning levels of each patient.

Qualifications

• A Bachelor's, Master's, or Doctor of Physical Therapy (DPT) degree from an accredited program is required.
• Current and unrestricted licensure by the Board of Physical Therapy to practice in California.
• Experience and willingness to conduct evaluations and therapy sessions both in-person and via telehealth where clinically appropriate.
• A sincere interest in treating a wide array of patients, demonstrating cultural competence and sensitivity.
