Position Overview

Alveo Aged Care is looking for a dedicated and skilled Physical Therapist to join our dynamic team at our rehabilitation facility located in Palmdale, California. In this full-time position, you will play a critical role in helping patients recover from serious injuries, surgeries, and illnesses. Work Environment

Located in the welcoming community of Palmdale, California, our rehabilitation facility offers a state-of-the-art setting designed for optimal patient recovery. Key Responsibilities

• Conduct initial assessments to evaluate patients' physical limitations, mobility issues, and overall functional abilities, and develop individualized treatment plans based on assessment findings.
• Design and implement therapeutic exercises tailored to improve patients' strength, range of motion, balance, coordination, and endurance, using various therapeutic modalities.
• Assist patients in relearning safe and efficient walking techniques post-injury or prolonged periods of immobility, ensuring a gradual and supportive transition to independence.
• Educate patients and caregivers on fall prevention strategies, helping to reduce fall risks through tailored interventions and training.
• Develop and facilitate pulmonary rehabilitation programs, aimed at enhancing the respiratory health and exercise capacity of patients with lung conditions.
• Collaborate with other healthcare professionals to provide holistic care, ensuring that patient treatment plans are cohesive and comprehensive.
• Support residents in improving their ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs), fostering independence in daily tasks.
• Incorporate cognitive rehabilitation activities into treatment plans, supporting mental acuity and engagement as part of the recovery process.

Patient Population / Client Focus

You will work primarily with older adults and individuals recovering from serious health conditions that require rehabilitative therapy. Many patients come to us after orthopedic surgeries, strokes, or debilitating illnesses. Your expertise and compassionate care will be crucial in helping them regain strength and functionality, ultimately improving their quality of life.

Qualifications

• A valid Physical Therapy license in California is mandatory.
• A Doctor of Physical Therapy (DPT) degree or an equivalent qualification from an accredited institution is required.
• Strong interpersonal and communication skills are essential to effectively engage with patients, families, and the healthcare team.
• Ability to develop and implement personalized, patient-specific treatment plans based on clinical assessments.
• A passion for enhancing patient recovery and improving daily living is important for achieving meaningful results.

Preferred Experience

• At least two years of clinical experience in a skilled nursing facility setting is preferred.
• Experience working within a multidisciplinary team is advantageous.
• Familiarity with rehabilitation technologies and methodologies that enhance patient outcomes will be beneficial.

Compensation and Benefits

We offer a competitive annual salary range of $90,000 to $100,000, commensurate with experience, along with a comprehensive benefits package. This includes:
