Exciting Opportunity for a Home Health Physical Therapist! Job Description:
- Receive and review physician therapy orders for Home Health and Hospice patients.
- Complete comprehensive patient evaluations and establish tailored treatment plans with both short-term and long-term goals.
- Provide skilled physical therapy interventions, including:
- Therapeutic exercises
- Stretching
- Mobility training
- Balance activities
- Pain management techniques
- Functional rehabilitation
- Assess home safety and recommend:
- Equipment
- Assistive devices
- Environmental modifications to promote independence and prevent falls.
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
- Educate patients, caregivers, and family members regarding:
- Treatment plans
- Home exercise programs
- Safety measures
- Expected outcomes
- Maintain accurate and timely clinical documentation, including:
- Evaluations
- Progress notes
- Discharge summaries
- Recertifications
- Resumptions of care
- Discharge documentation
- Collaborate and communicate effectively with:
- Physicians
- Nurses
- Therapists
- Other healthcare professionals involved in patient care.
- Travel to patient homes and other care settings to provide therapy services.
Requirements:
- Current PT licensure in the state of Montana
- Valid driver's license
