The LPN will provide nursing care to patients, support their health needs, communicate with families and the care team, and complete required documentation. The position also includes participation in an on-call rotation for occasional clinical needs.Who You Are:Graduate of an approved school of practical nursing, currently licensed as a Licensed Practical Nurse in the State of IndianaTwo (2) years of experience in home health care or related health field (Preferred)Car with required proof of insurance coverage and an Indiana Driver's licenseKnowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentationCare Plus Homehealth Care is an equal opportunity employer.
