Integrated Behavioral Health Care Manager

Molina Healthcare is seeking a care coordination professional in the Miami area to advance integrated member care. You will assess behavioral health needs, develop care plans with multidisciplinary teams and conduct visits as needed. You will monitor progress, coordinate services, and engage in care transitions, while working with the member and care network to achieve outcomes.
