LPN Behavioral Health Care Manager - Collaborative Care

Infinite Medical P.C. is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within our Collaborative Care Model. This in-office, non-face-to-face role supports residents in skilled nursing and long-term care facilities through chart review, screening, care planning, and team collaboration.
