Hospice Licensed Practical NurseSalary: $31.25-$34.13/hr BOESchedule: Full time | M-F| 8:00AM-5:00PM +(on-call rotation)Location: Warren County/ North Hamilton County, OhioRequirements of the Hospice LPN role:Licensed Practical Nursing DegreeBLS CertifiedA minimum of 1 year of Clinical experienceHospice experience preferredValid Drivers License and Auto insurance CoverageReliable transportationAbout the Hospice LPN role:Provide skilled nursing care under the supervision of an RNAssist Registered nurses in carrying out the plan of care.Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.Assist reinforcement of patient and family education regarding end-of-life care.Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.Keep the RNCM informed of changes noted during the process.Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.Why work with Care 360 Hospice:Medical, dental, and vision health InsurancePaid Time Off401(k) MatchingLife InsuranceGrowth opportunitiesWe appreciate your interest in our Hospice Licensed Practical Nurse.
