Nurse Practitioner (PRN) - In-Home Health Assessments

Role Overview Seeking a Nurse Practitioner to perform comprehensive health assessments in members' homes or facilities, reviewing health history, medications, social factors, and risks to improve care quality and identify hidden conditions. Key Responsibilities Conduct adult/geriatric assessments, documenting medical history, diagnoses, and treatment plans Administer and order point-of-care and lab tests Collaborate with physicians, case managers, social workers, and caregivers Coordinate follow-up and patient education with primary care providers Provide services at members' residences, including homes and skilled nursing facilities Qualifications & Skills Master's degree or equivalent, current NP licensure, and certification in relevant specialties Minimum 1 year NP experience, BLS/ACLS/CPR certified Ability to travel extensively, with valid driver's license and insurance Strong computer skills, excellent communication, and ability to work independently Additional Info Flexible scheduling with competitive pay based on assessments completed Support for licensure in multiple states and career advancement opportunities Commitment to diversity, inclusion, and employee growth #J-18808-Ljbffr
