Children's Care Coordinator

Care Coordinator New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast, serving 15,000 children and adults annually through inpatient crisis services, a 24-hour help line, mobile response team, and outpatient programs across Indian River, Martin, Okeechobee, and St. Lucie counties. We also assist 22,000 students in area schools and work closely with courts, law enforcement, jails, and hospitals to improve community health. Responsibilities Provides time-limited care coordination for 12-15 identified individuals using internal utilization reviews and SEFBHN high-utilizer recommendations. Coordinates services until the individual is connected to appropriate care and progress toward Care Coordination goals is achieved. Ensures individuals are engaged in their current setting (e.g., crisis stabilization unit, SMHTF, homeless shelter, detoxification unit, addiction receiving facility) to establish a warm hand-off. Maintains an internal process that ensures on-call services are available 24 hours, seven days a week. Models, coaches, and supports shared decision-making in care planning and service determination with the individual and family members (where applicable) emphasizing self-management, recovery, and wellness, including transition to community-based services and/or supports. Works with SEFBHN to develop diversion strategies to prevent individuals who can be effectively treated in the community from entering SMHTFs. Qualifications Master's degree in counseling, social work, psychology, or a related human services field and two years of full-time or equivalent experience working with adults and children at risk. Professional license preferred. Must pass Level 2 Background through DCF and AHCA Clearinghouse. Valid and current Florida Driver’s License with no more than 6 points in the past five years.
