RN Case Manager

We are seeking a Motivated RN to join our team in Utah County as a Case Manager to assist with performing 60 day recertification assessments, admission visits, resumption of cares, CNA supervision, routine nursing visits and care coordination. This person will have a clear path of career trajectory and growth within Amazing Care alongside hands‑on training. The schedule is flexible! Responsibilities Hourly pay for case management time Pay per visit when in the field performing visits in the home Spanish Fork - Lehi areas Administer medication, treatments, and therapiesas prescribed bythe physician. Monitor patient's condition and report any changes to the healthcare team. Assistwith activities of daily living and provide emotional support to patients and their families. Collaborate with other healthcare professionals to ensureoptimalpatient care. Maintainaccurateand complete medical records using web-based electronic medical records system. Qualifications Active RN License. Previous Home Health experience preferred. Current BLS Certification. Valid Driver's License and Vehicle Insurance. Strong time management and prioritization skills.
