Location: Springfield, VT Job Id: 226 # of Openings: 0 2026 VT Best Places to Work Overview Senior Solutions (Area Agency on Aging in southeastern Vermont) is seeking a Case Manager to work with older adults and/or people with disabilities in the Springfield area and surrounding areas using a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required. Qualifications Bachelor’s degree in human services and prior case management experience preferred. A combination of relevant education and experience may be substituted for a degree. Must have reliable transportation and a valid driver’s license. Passion for working with seniors or adults with disabilities. Responsibilities Collaborate with other community partners to create a person-centered plan of care. Monitor and reassess program service plans as required. Maintain a working knowledge of services and programs available to older Vermonters and people with disabilities. Assist, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc. Ensure that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
