Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaint in accordance with RN scope of practice and facility policies.
• Develop and prioritize individualized patient care plans in coordination with the inter-professional team, patient, and family, and fully implement care therapies and procedures within scope of practice.
• Recognize, respond to, and communicate changes in patient condition; conduct and document ongoing assessments and responses to procedures and therapies using the nursing process.
• Facilitate patient admission, transfer, and discharge activities, including timely execution of discharge orders, discharge teaching, and coordination with Case Management.
• Delegate tasks appropriately and monitor accuracy and completeness of delegated patient care provided by other team members.
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing.

• Active, unrestricted Registered Nurse (RN) license in the applicable state.
• Basic Life Support (BLS) certification required.
• Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN).
• Ability to communicate and collaborate effectively with all members of the care team.
• Proficiency with or ability to be trained on computer systems and clinical applications.
• Ability to assess age-specific patient safety issues and implement appropriate safety measures.

• Previous clinical nursing experience in a hospital or acute care setting.
• Bachelor of Science in Nursing (BSN).
