Salary: $29.00 - $31.00 hourly (DOE) | Hourly, Non-Exempt | Full-Time | Location: Puyallup, WA Medical, Dental, Orthodontic, Vision, and Rx — minimum 80% of employee monthly premiums covered; HSA; dependent coverage available at employee’s expense Employer-sponsored Life, AD&D, and Disability Insurance Voluntary supplemental plans: Accident, Cancer, Critical Illness, STD, Identity Protection, and more Time Off Accrue up to 132 hours (16.5 days) of PTO in your first year, based on FTE status 8 paid holidays for full-time employees 401(k) with employer match on first 4% Work-Life Quality Monday–Friday | Typical hours 8:00 AM – 4:30 PM; no nights or weekends Core Values that promote work-life harmony A collaborative, team-driven culture that promotes recognition and celebrates everyday wins What You Bring Credentials: Active MA, LPN, or RN license/certification in Washington State Technical: Computer savvy; intermediate proficiency in MS Suite — Word, Teams, Outlook, and Excel Communication: Strong verbal and written communication skills; professional phone etiquette and excellent customer service Organization: Excellent attention to detail, strong time management, and ability to prioritize time-sensitive situations with urgency Interpersonal: Strong interpersonal skills with the ability to communicate effectively across a variety of people and settings Adaptability: Ability to multitask while providing compassionate patient care in a fast-paced environment Leadership: Leadership qualities with the ability to work well within a team of healthcare professionals Character: Self-motivated, punctual, dependable, trustworthy, honest, and professional with a positive attitude What You’ll Do Maintain care coordination dashboards, patient lists, and tracking for CCM/PCM programs Track referrals, authorizations, and follow-up appointments across specialty and post-acute services Conduct non-clinical patient and family outreach, including welcome calls and appointment reminders Coordinate communication between patients, facilities, Care Managers, and providers Gather, upload, and organize medical records, wound photos, labs, and imaging reports Support scheduling for follow-up visits, interdisciplinary rounds, and provider appointments Complete clerical tasks related to Care Management, including outreach, scheduling, and documentation Monitor admissions, discharges, and census changes from skilled nursing and post-acute facilities Prepare patient summaries, reports, and materials for care team meetings Escalate clinical concerns promptly to a Care Manager or provider Participate in required training, meetings, and compliance activities Ready to Make an Impact?
