Description

• Conduct comprehensive evaluations of patients in either inpatient or outpatient settings and curate personalized treatment plans

• Work collaboratively with a multi-disciplinary team, including nurses, physicians, case managers, and rehabilitation specialists, to maximize patient outcomes
• Educate patients and their families about therapy goals, discharge preparations, and home exercise routines
• Diligently document evaluations, treatment sessions, and patient progress according to facility standards and regulatory requirements
• Engage actively in care team meetings and interdisciplinary discussions to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) from an accredited program is required

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological or geriatric therapy) are advantageous but not mandatory

• Prior related experience is strongly desired
