Remote Care Coordinator: Health Outreach & Home Visits

EPITEC

A health care services provider is looking for an Outreach Coordinator to support members in managing their health care needs. This remote role, with occasional travel, involves conducting health assessments, performing outreach for health programs, and coordinating member services. Candidates must have a degree in Social Work or Psychology, or relevant experience in managed care. Strong communication skills are essential.
