Responsibilities
- Provide direct, compassionate nursing care to residents within a long-term care setting, under supervision as required.
- Monitor vital signs, administer medications and treatments per the care plan, and document actions accurately in the chart.
- Assist residents with activities of daily living and mobility, while promoting safety, dignity, and comfort.
- Recognize and report changes in resident condition to the supervising nurse and physician as appropriate.
- Collaborate with the interdisciplinary team to implement care plans and adjust interventions as directed.
- Maintain infection control practices, safety protocols, and patient privacy in all care activities.

Requirements
- Active LPN license to practice in New Hampshire or eligibility for licensure in NH
- Graduation from an accredited practical nursing program (LPN)
- Minimum 1 year of related experience in long-term care or skilled nursing preferred
- Ability to work the listed shift and perform duties in a long-term care setting
- Strong documentation and communication skills
