Family-Centered Treatment Specialist: In-Home & Community Care

Health Connect America

Health Connect America is seeking a Family Centered Treatment Specialist to deliver FCT services to children, adolescents and families in home and community settings. The role requires adherence to the service model, ongoing service planning with the Treatment Team, and coordinating discharge and aftercare needs. A Bachelor’s degree in human services is required, with two years’ experience and preferred FCT certification. Ohio licensure considerations apply. #J-18808-Ljbffr Health Connect America
