(RN) Registered Nurse - Home Health - Raeford, North Carolina

Step into a vital nursing role providing in-home care to patients throughout your community. This full-time, 13-week contract position offers the chance to deliver one-on-one patient care where it matters most directly in patients' homes. You'll work Monday through Friday, from 8:00 AM to 5:00 PM, managing a caseload of approximately 35 patient visits per day (30 points per week), supported by advanced electronic documentation and a collaborative clinical team.

Key Responsibilities:

• Conduct thorough OASIS assessments and develop customized care plans.
• Deliver skilled nursing care, including wound management, IV therapy, medication administration, and chronic disease education.
• Monitor patient progress, identifying changes and communicating promptly with multidisciplinary care teams and physicians.
• Provide education to patients and families about disease processes, medications, and preventive strategies.
• Maintain precise and timely documentation using Home Care Home Base (HCHB) software.
• Collaborate closely with PTs, OTs, medical social workers, and home health aides to ensure comprehensive, coordinated care.
• Attend case conferences, staff meetings, and participate in ongoing training.
• Adhere to agency and regulatory standards for high-quality patient care and accurate documentation.

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active RN license in good standing (state of assignment)
• At least 1 year of recent, relevant home health nursing experience with field-based care preferred
• Demonstrated proficiency with HCHB and experience completing OASIS documentation
• Reliable transportation, a valid drivers license, and auto insurance
