Location: Patrick, Henry, Pittsylvania & Franklin Counties, VA

Schedule

• Monday–Friday | 8:00 AM – 5:00 PM
• On-Call: 1 week per month (after hours)
• Hours Guaranteed: No

License Requirements

• Active Virginia RN license (Compact license accepted)

Productivity

• 30 points

Mileage Reimbursement

• $0.43 per mile

Job Summary

We're seeking an experienced Home Health Registered Nurse to provide comprehensive patient assessments, coordinate individualized care plans, and collaborate with physicians and interdisciplinary teams to improve patient outcomes. You'll serve as a patient advocate while ensuring high-quality, cost-effective care in the home setting.

Key Responsibilities

• Complete comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, social workers, and other healthcare professionals.
• Manage care transitions and connect patients with needed community resources.
• Monitor patient progress and adjust care plans as needed.
• Maintain accurate clinical documentation and outcomes tracking.
• Promote wellness, prevent complications, and advocate for patients throughout their care.

Qualifications

• Associate's or Bachelor's degree in Nursing.
• Current, unrestricted RN license in Virginia (Compact accepted).
• Home health experience required.
• Case Management certification preferred.
• Strong clinical assessment, communication, and critical thinking skills.
