License Requirements: Virginia License (compact license accepted)
Productivity Requirement : 30 points
Mileage reimbursement: 43 cents
Counties covered : Patrick, Henry, Pittsylvania, Franklin
Required Skills: Home health, HCHB, Oasis, wound care, wound vac, IV, lab draws

Duties :

• Performed comprehensive assessments of high-risk patients to identify clinical and care needs.
• Developed and implemented individualized care plans in collaboration with primary care providers.
• Coordinated care transitions with physicians, social workers, discharge planners, and interdisciplinary teams.
• Arranged non-medical support services, including housing and transportation, to improve treatment compliance.
• Monitored patient progress and adjusted care plans to ensure appropriate, cost-effective care.
• Maintained accurate documentation of clinical, functional, and financial outcomes.
• Promoted health education, disease prevention, and early intervention to improve patient outcomes.
• Served as a patient advocate by protecting privacy, ensuring confidentiality, and coordinating specialty services as needed.
