Clinical Pharmacist

We Are Hiring Location: Tulsa, OK Facility: Ascension St. John's Medical Center Department/Specialty: Pharmacy Ambulatory Clinics Schedule: FT, Monday-Friday, 8am-430pm or 830am-5pm Relocation Assistance available What You Will Do Enhance Patient Outcomes: Pro-activelymanage chronic diseases (e.g., diabetes, hypertension) to prevent complications and improve overall quality of life. Optimize Pharmacotherapy: Perform comprehensive medication management, interpret labs, and safely adjust dosages under collaborative practice agreements. Boost Medication Adherence: Identify financial or insurance barriers and provide targeted education to streamline therapy and ensure access. Support the Care Team: Collaborate directly with providers to streamline workflows and ease provider burnout. What You Will Need Licensure / Certification / Registration: Pharmacist credentialed from the Oklahoma State Board of Pharmacy required. Education: Bachelor's or advanced degree in Pharmacy required.
