Key Responsibilities

• Provide skilled therapy services in patient homes under the direction of a supervising Physical Therapist
• Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
• Monitor patient progress and communicate changes to the supervising Physical Therapist
• Encourage patient participation and reinforce established therapy goals
• Complete timely and accurate documentation in an EMR system
• Collaborate with therapists, nurses, caregivers, and other care team members

Required Qualifications

• Current Colorado Physical Therapist Assistant (PTA) license in good standing
• Graduate of an accredited Physical Therapist Assistant program
• Valid driver's license, reliable personal transportation, and auto insurance
• Strong communication and documentation skills
• Ability to work independently and adapt professionally in a home-based setting

Preferred Experience / Skills

• Prior experience in home health or community-based therapy settings
• Familiarity with electronic medical record (EMR) documentation systems
• Experience working with diverse patient populations across varying levels of acuity
