MDS Coordinator

Accela Rehab & Care Center at Manalapan is seeking a knowledgeable and detail-oriented MDS Coordinator to join our clinical team. The MDS Coordinator plays an essential role in coordinating the resident assessment process, ensuring accurate and timely completion of MDS assessments, and supporting individualized care planning and quality outcomes.

The ideal candidate is an experienced RN with strong organizational and communication skills and a solid understanding of the MDS process in a skilled nursing or long-term care environment.

Responsibilities

• Coordinate and oversee the facility's MDS assessment process.
• Conduct comprehensive resident assessments and gather information from nursing, therapy, dietary, social services, and other interdisciplinary team members.
• Accurately complete and submit MDS assessments within required regulatory timeframes.
• Develop and maintain individualized care plans based on resident assessments and identified needs.
• Coordinate with the interdisciplinary team to ensure resident care plans accurately reflect current conditions and services.
• Monitor residents' progress and ensure assessments and care plans are updated as required.
• Maintain compliance with applicable MDS regulations, guidelines, and facility policies.
• Review documentation for accuracy, completeness, and consistency.
• Identify discrepancies or missing information and work with appropriate departments to obtain clarification.
• Participate in care plan meetings and communicate resident needs and changes to the interdisciplinary team.
• Support quality improvement initiatives and assist with identifying opportunities to improve resident outcomes.
• Maintain accurate MDS-related records and documentation.
• Assist with regulatory surveys and provide requested MDS and clinical documentation.
• Promote resident-centered care, comfort, dignity, and safety.
• Work collaboratively with nursing leadership and other departments to support quality care and resident satisfaction.
• Stay current with changes to MDS requirements, regulations, and best practices.

Qualifications

• Current New Jersey RN license required.
• Previous MDS Coordinator experience strongly preferred.
• Experience in a skilled nursing, long-term care, or short-term rehabilitation setting preferred.
• Strong understanding of the MDS assessment and care planning process.
• Excellent organizational and time-management skills.
• Strong attention to detail and accuracy.
• Excellent written and verbal communication skills.
• Ability to work effectively with an interdisciplinary team.
• Ability to prioritize multiple responsibilities and meet regulatory deadlines.
• Strong knowledge of resident-centered care and long-term care practices.
• Professional, dependable, and compassionate approach to resident care.
• Ability to maintain confidentiality and handle sensitive resident information appropriately.
