The YoloCares Clinical Team The Clinical Manager will join a highly skilled, compassionate interdisciplinary clinical team dedicated to providing patient- and family-centered hospice care. YoloCares’ clinical team includes registered nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who work collaboratively to support patients and families through every stage of the hospice journey. The team is grounded in clinical excellence, mutual respect, and thoughtful coordination, with a shared commitment to dignity, comfort, and individualized care. Position Summary The Clinical Manager supervises skilled nurses, hospice aides, social workers, and spiritual care coordinators. They also participate in administrative after-hours on-call rotation and assist the Chief Clinical Officer (CCO) in daily clinical operations. Responsibilities Triage patient and family calls in a timely manner. In coordination with the CCO, provide assistance to RN Case Managers as census dictates, including taking on a caseload when the census fluctuates. Support the interdisciplinary team through effective collaboration and planning. Demonstrate understanding of patient eligibility requirements for hospice care, including working knowledge of guidelines for determining terminal status based on Local Coverage Determination’s (LCD’s) Medical Review Policy and Palliative Performance Scale (PPS). Assure proper maintenance of clinical records in compliance with Joint Commission Standards. Create and maintain a monthly schedule for all clinical on-call/after-hour services. Oversees hospice aides and RN case managers’ schedules ensuring patient needs are covered; approve time off requests in time management system. Participate in the orientation program for new hospice staff, ensuring direct reports receive proper orientation. Participate in quality assurance/performance improvement activities as appropriate. Conduct monthly ride‑along visits with clinical staff to evaluate performance, reinforce best practices, and ensure full compliance with Perfect Visit standards. Demonstrate understanding of hospice resources and allocate consistently and appropriately. Demonstrate knowledge and awareness of palliative care principles. Demonstrate the ability to problem‑solve and facilitate patient/family decision‑making. Assure compliance with all local, state, and federal laws regarding licensure and certification of agency personnel. Stay informed about changes in the field of nursing and hospice care; share information with appropriate agency personnel. Participate in shared administrative after‑hours clinical administrator on‑call rotation. HIPAA “Minimum Necessary” Access : This position requires access to all clinical and financial components of a patient’s medical record. Qualifications Current California license as a registered nurse (RN) in good standing with the State Board and CPR certification. Bachelor of Science in Nursing from an accredited school of nursing. Minimum of three (3) years of nursing experience, preferably in a hospice, home health agency or acute, including a least one (1) year in hospice or home care. Minimum of one (1) year of supervisory experience in a healthcare setting. Experience supervising the work of professional and administrative personnel; prior supervision of clinical healthcare staff preferred. Clinical expertise in the care of terminally ill patients, including pain and symptom management. Knowledge of palliative and/or terminal care, including effective pain and symptom control techniques and preparing patients and families to cope with the psychosocial aspects of terminal illness and end of life. Excellent observation skills. Ability to communicate effectively in English, both verbal and in writing; Spanish language skills are a plus. Ability to take initiative and make appropriate independent work decisions. Knowledge of group processes and ability to facilitate an interdisciplinary team approach to patient care. Conflict‑management skills that foster open communication, equal respect for self and others, and accountability for one’s actions. Ability to manage frequent interruptions, prioritize completing responsibilities, and complete multiple duties and projects, including under tight deadlines. Knowledge of Medicare Hospice Conditions of Participation, Medi‑Cal hospice services, private insurance hospice services, and California hospice licensing requirements preferred. Computer literacy, including proficiency in internet use and database systems; experience with Excel spreadsheets is preferred. Willingness and ability to flex or add work hours to meet scheduled on‑call shifts during evenings, weekends, and holidays. Ability to meet agency background screening requirements. Physical and Other Job Specifications Able to hear, speak, read, write, and type well enough to perform required job duties. Ability to safely lift medical equipment (e.g. a wheelchair) weighing up to 35 pounds and to assist family/caregivers with transferring and ambulating patients as safety requires; considerable walking and standing may be required. Must have visual acuity and hearing to perform required nursing tasks and duties. Ability and willingness to maintain and drive a personal vehicle. Must have a valid California driver’s license and state‑required automobile insurance if driving on YoloCares business. Physically able to make home visits and perform CPR.
