Job Title: RN Home Health Profession: Registered Nurse Specialty: Home Health Duration: 260 days Shift: 2x3 days Hours per Shift: Not specified Experience: Two years of experience in a home setting within the last fifteen years License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: Recent experience conducting comprehensive assessments of patients Ability to work with technology in the workplace Excellent organizational, interpersonal, written, and verbal communication skills Ability to communicate and work effectively with multi-generational consumers Ability to demonstrate sensitivity to the concerns of others Ability to perform comfortably in a fast-paced, deadline-oriented work environment Strong ability to work independently Description: This position is an urgent need and involves performing initial in-home pre-evaluation of patients. The evaluation includes assessing health status, strengths, care needs, and preferences. This guides the development of individualized long-term care service plans. You will review consumer medical documentation and relevant health referral forms. Data must be entered into an electronic evaluation form and transmitted as required. Document any concerns or conflicting information that arises during the evaluation process. Emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Discuss healthcare options, supports needed, service vendor options, and waiver options with consumers. Initiate communication for enrollment into a managed care health plan. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data to support outcome measurements. Maintain comprehensive knowledge of community resources and payor requirements. Meet all standards established for this position as outlined in the corresponding performance criteria. Perform other duties as assigned by the Regional Nurse Manager or project management.
