Physical Therapist Assistant (PTA) - Consistent PRN

Location: Energy, IL
Facility: Helia Healthcare of Energy
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) for a consistent PRN opportunity at Helia Healthcare of Energy in Energy, Illinois.

This position offers a predictable schedule of 4-5 days per week while providing meaningful patient care in a supportive, collaborative skilled nursing facility where your clinical expertise makes a daily impact.

As a Physical Therapist Assistant (PTA) , you will provide skilled physical therapy treatments under the direction of a Physical Therapist while helping patients improve mobility, strength, balance, and functional independence.

• Provide skilled physical therapy treatments according to individualized patient care plans.
• Assist patients with therapeutic exercises, mobility training, balance activities, and functional activities.
• Monitor patient progress and communicate changes to the Physical Therapist.
• Document treatment sessions accurately and timely.
• Collaborate with Physical Therapists, nursing staff, caregivers, and interdisciplinary team members.
• Educate patients and families on mobility strategies and safety techniques.
• Provide compassionate, patient-centered rehabilitation services.

• Graduate of an accredited Physical Therapist Assistant program.
• Current Illinois Physical Therapist Assistant (PTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality patient care.
